እኔ ከዚህ በላይ ሙሉ ስሜና አድራሻዬ የተመዘገበ የኢትዮጵያ ኦርቶዶክስ ተዋሕዶ ቤተክርስቲያን አማኝ ስሆን ቅዱስ ሲኖዶስ የሚያወጣውን መንፈሳዊ መመሪያና የቤ/ክኒቱ አስተዳደር የሚመራበትን ሕግ /ቃለ አዋዲ/ ተቀብዬ በቃለ አዋዲው አንቀጽ 61 ላይ የተጠቀሰውን የምዕመንነት መብት እና ግዴታ በመረዳት በደብሩ የሰበካ ጉባኤ አባልነት እንድመዘገብ እጠይቃለሁ፡፡ እስማማለሁ::

I, being a member of the Ethiopian Orthodox Tewahedo Church (EOTC) will follow the directions given by the Holy Synod and the hierarchical administration. I fully accept and will abide by the Church’s Proclamation /Kale Awadi/, I also read and understand the rights and obligations of the Church members stated on Article 61 of the EOTC Proclamation /Kale Awadi/ and hereby request for membership of the parish church.
